Post-Operative Care

People heal better at home.

Equinoxe LifeCare offers in hospital and at home post-operative supportive care to ensure that you are comfortable, and get all the attention you need to facilitate your recovery. We can supervise your transition home from the hospital, make sure your home is properly equipped, and provide whatever level of care you need in the days or weeks following surgery including Registered Nurses who can be on call 24 hours a day.

Senior Care

As our parents and grandparents age, their desire to remain independent and living at home is very often strong, regardless of our reservations about their safety, security and ability to fully care for themselves.

We can’t always be there. And we don’t want them to be alone.

From a few hours a week, to 24 hours a day, our Care Coordinators will schedule R.N.’s, Nurse’s Aides or Companions to help your aging family members with whatever they need to overcome their challenges, eat properly, go for walks, attend activities and doctors’ appointments… and enjoy their lives to the fullest.

Alzheimer’s and Dementia Care

As your family copes with this difficult situation, your Equinoxe LifeCare Coordinator will be there to help you get all the supportive information you need, and to create a treatment approach to address all the physical, emotional and psychological needs of your loved one.

We can configure living spaces to facilitate independence, and provide assistance with medication monitoring. Our goal is find strategies to keep those with dementia at home as long as possible… safely, comfortably, and with dignity.

Chronic Disease Management

If you or someone you love is living with a chronic disease at any age, you know how valuable it is to have reliable, personalized care to make life easier. A skilled and compassionate Nurse’s Aide or Companion can make all the difference for someone with challenges such as limited mobility, pain, anxiety and depression… all associated with chronic health problems.
